In addition, permission is granted for use of my name & voice for promotional purposes. Further, I understand that I may win only once in a 30 day time period. A violation of the 30 day contest rule may result in the forfeit of prize(s).

PLEASE NOTE:

Daddy's does not mail prizes no exceptions! If you live out of the state of NH, your prize will be sent to your nearest
Daddy's location for pick up.
Prizes must be picked up within 30 days or the prize is forfeited.
Winner must show proper ID to retrieve their prize.
If a prize is valued over $600, winner must provide Social Security Number.
